Steps: How can we find the bug ourselves? Open the game offline
What happens when the bug occurs? I open my game offline since that's how I always play and when I get to main menu, a pop up saying "connection is required, you are currently updating the game. Please reconnect to continue updating game" appears
What do you expect to see? Sims delivery express items downloaded since I already opened my game online in order to download, and it said it completed the download and the items appear when I'm online.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Yes
Please describe the patch or change you made. It was the most recent patch update when the first SDX item was released
I opened my game online in order to update and download the SDX items. Once that is complete I restart my game as prompted l, then close it when done.
I then go offline and restart my game to generate a new sims folder to avoid issues, something I always do after an update. Once I get to main menu the pop up saying I'm still downloading content appears. I've even tried restarting offline without regenerating my sims 4 file after trying to download the SDX again and still the pop up appears. I've repaired my game, updated origin, even had to redownload origin and still the problem persist.
Whether I have my mods and cc in game or not, it continues and the items suddenly no longer exist in my game at all. The pop up continues to appear every hour or so I'm ingame if I decided to just give up on the download.
I'm still experiencing this issue. I honestly can't be the only person 8nnthe whole dims community experiencing this. There is seriously no solution or commentary. I've been having this issue since the beginning of the SXD and it persists even today. I've been reaching out even on twitter. This is honestly getting to me.
I started having this problem today.
The game crashed, and when I opened Sims 4 next, I had an error message popping up in the right top corner. Sims Delivery Express Connection Error. When I try to open the game I was playing when it crashed, I'm notified that the game has items placed in world that I don't have downloaded via Sims Delivery Express. Clearly I once had said items. I don't know where to look to see if I still have said items (I wouldn't even know which items were missing if I were to load a different game).
I don't have any EP/GP/SP/Ks. I removed the mods I had, and still had the same problem.
I have restarted the game multiple times along with my computer.
I went into the Game Options menu on the Sims Delivery tab and attempted to check for updates. On that tab, it has a big message saying that the download stopped with a button to "resume download". Every time I click the button, it attempts to connect to check for updates for a second and then stops. My computer is connected to my home network which has active internet connection. I do have a VPN, but I have tried this with VPN both on and off.
I had no indication of what caused the crash. No error message after, either. Just a clean, sudden software ended.
In the bug report for the Delivery Express connection error mentioned above, I read that removing the "content" folder in your "documents" would prompt the Delivery Express stuff to redownload. I don't have a "content" folder in my "documents" folder, nor in "documents > electronic arts > the sims 4".
I have Windows 10. I don't play Sims 4 every day, but I have played before today and since I last installed any updates for OS or anything else. Nothing has changed to my knowledge between when I last played Sims 4 and when the crash happened.

I'm sorry you've had this problem for so long. I imagine it's very annoying. I will link that other bug report. The two errors may be related to one problem with the Delivery Express system.

Mostly for others who have this or similar issue and find this posting:
If you are one Windows 10, check how your OneDrive auto back up is set up. Based on some experiments, your OneDrive may have the still messed up files, and may automatically redownload these to your PC. Make sure the OneDrive files are deleted/removed as well as the files on your PC.
Hai! I know this is a really late reply but I was having this problem, I managed to fix it by removing everything from my mods folder, and then once it was finished I just put everything back into the mods folder.
Please use the tech forums if you have further issues with SDX not downloading
