There should at least be one Doctor in the .not in world section pick a house to put one of the sims in, have that sim join the doctor career, than evict from house and remove played household. This will allow the game to promote that sim to doctor.

Clinic should have 2 doctors, 1 receptionist, and 1 nurse during the shift. Make sure pregnant sim is in good mood, the pregnancy tends to reset if they are stressed or angry. If sim is tense or uncomfortable or angry the receptionist will ignore the sim. Also make sure all there needs are in the green like bladder and hunger. Sometimes if go to the hospital as soon as the in labor moodlet appears it may appear that it is not working, walk around a bit go to the bathroom, your get a Oh, what just happened popup, I think my water broke, than checking in, the Doctor should come running from outside with the freakout thought bubble of a baby passapher over his head. meanwhile the Dad will be running around freaking out over the birth. He may than say in a popup I hope I am up to this. The labor thing may start and you can go to the hospital any time during the labor process but the hospital won't know your in labor until your bladder goes to green again.

Edit: For people that want to modify the hospitals, there needs to be at least two surgery tables and two doctors in case the sim in labor has identical twins they will both need space to come out at the same time.  If you are going to have identical twins you will see both doctors head for both tables one baby will spawn at the table the one giving birth and the other will spawn at the empty table.

Failure to provide both tables could cause issues such as this if your sim is trying to deliver identical twins, I popup should come up saying you have twins if there is two options in the dialog box for names of both babies than its identical twins, if you get the babies in separate dialog boxes, they are not identical twins.

Edit: I have fixed some mistakes I noticed.

All of my Sims needs were green. She was uncomfortable because of the labour moodlet, but even when the countdown had reached 0 minutes it took several sim-hours before the baby was born.

I have not modified the hospital in any way. Except for the reseptionist, there was one nurse who is a ghost. No other staff like doctors or nurses where at the hospital.

could you try this (worked for me although the doctor was a ghost)

• Reload your game and don't load the Sim at the hospital but one of the elders at home
• Wait a couple min
• Switch to the pregnant Sim.
• This immediately made a ghost doctor spawn for me.
