the bug only exists if the wedding cake topper is added after the sims have married, To be able to take the first slice wait till the caterer makes the cake or make the cake yourself add the topper then get married after that the option should show up, you can clear the marriage by clicking on shift and the sim, then marriage, after that make the cake and get married ,
