@Sassyone59 Hi! I've had this issue with and without mods, so I don't think it's caused by a mod. Have you tried the suggestion to hire a nanny and have them introduce the infant? It's not ideal but at least a workaround. In my game, I was able to have one parent introduce the other.
Since you mentioned having mods, there is a set relationship option using MCCC if you have that. With the infant as the active sim, you would click on whatever parent needs intro then choose the MCCC option -> Relationships -> Set Active Relationship Levels -> Set Active Frriendly -> (whichever friendly level you prefer; Acquaintances = minimal green status bar and each level in between adds more green through True Friends = full green status bar). I haven't tested it yet but just giving them some Friendship level should remove the need for introduction.

@Sassyone59 - No need to skip the infant stage. Just hire a Nanny on your phone and ask them to introduce you. It's possible another household member who didn't get hit with the glitch, or even a Servo with the Nannybot Trait Chip could do it too. Being introduced by a third party will break through the glitch. I you don't need the Nanny's service, you can just dismiss them once the introduction is made.
@RASMUS_MASSIE It has worked for me several times. Here's what it looks like for my parent sim right after age up to infant.
Here is what it looks like after I used MCCC to adjust friendly relationship level to "Friends".
The method I described in my post (#110) also works for toddlers who can't interact with extended family like grandparents, aunts and uncles, etc.
(Apologies for those who don't use mods. This is a frustrating bug for sure. At least there are some workarounds like hiring a nanny.)
