This is a funny but annoying thing. I have merfolk leaving instruments littered on the beaches that never disappear.
I suspect it happens via rotational play, and I definitely noticed it more after Likes and Dislikes were introduced to the game.
My merman, Dylan Zalrian, he's a Music Lover, Creative, and Child of the Ocean when he's a young adult. When I had him like playing the Guitar, it became something he did when I was playing different households. I actually love seeing him play the Violin and Guitar, it's what he was born to do. The problem is that he doesn't always put the item back into his inventory.
Sims don't play instruments as NPCs normally, not without a busking station. For Dylan, it's a very different behavior. He's not playing an instrument to busk for money.
How this happens:
He has the Guitar or Violin in his inventory when I rotate out of the household. Dylan is more likely to use the instruments he has on him, but this doesn't exclude less creative sims.
When I play a different household, Dylan has the instrument in his inventory. So, an autonomous behavior he has is playing the instrument.

Create a Sim
Make that household a Played Household
Create another Sim in a seperate Household
Once Sim A and Sim B become friends, have Sim B invite Sim A with an instrument over to a lot without much to do
Sim A will play the Instrument if their skill level is high and they like playing said instrument

A good place to do this might be while on Vacation in Granite Falls. Granite Falls does not have a lot to do without changing the lots. I've had Dylan play the Violin for multiple Sim Days straight.
The above gets sims to do the behavior that leads to the bug.
After some time, the bug happens when the sim finishes playing the instrument.
In Sulani, I've had Lir Kailani play the Guitar on the Beach. That's really cool, but the Guitar ended up permanently in the Open World. That's where things get strange. The instrument is a part of the open world, not tied to a lot or a sim.
There's actually a bug with Landfill Lot Challenges that creates a similar effect. The Landfill Lot Challenge makes sims leave broken appliances on a lot. But, many people don't know that it's possible for sims to leave said broken appliances in the open world just outside the borders of the lot. Essentially, the dumped appliances become stuck in the open world. Trying to edit them out of the Open World while editing the lot is impossible.
I discovered this when I played a Merman Vet trying to breathe life into a worn down Brindleton Bay. I ended up having to restart the save because a random townie left a broken fridge outside the pet hospital. I couldn't delete it. I couldn't scrap it. I couldn't do anything with it. If I repaired it, that didn't do much good when it rained. I never use the Landfill lot challenge specifically to avoid having random junk stuck in the open world that I can't remove.
