Hello! I'm also experiencing this bug, it seems to have happened after the latest infant update / growing together pack. I hope I'm doing this report thing correctly, I've never had to post on the forum before. I have attached a few screenshots to hopefully help the team understand what I'm seeing on my end.

Which language are you playing the game in? English
How often does the bug occur? Every time (100%)
What is your current game version number?  1.96.397.1020
What expansions, game packs, and stuff packs do you have installed? Expansions - All  *Game - All except Star Wars *Stuff - Spooky, Luxury Party, Moschino, Tiny Living, Laundry Day, Vintage Glamour, Cool Kitchen, Perfect Patio, Movie Hangout, and Fitness *Kits - Blooming Rooms, Courtyard Oasis, Throwback Fit, and Carnaval Streetwear
Steps: How can we find the bug ourselves? Have a young adult sim that has parents. Have young adult sim have a child with another sim. Marry a sim who is not the other parent to the new infant sim. Have new spouse adopt infant sim. After adopting, check the family tree, CAS, and relationship info panel and see that it has now made it so sim 1 (sim that had baby) and sim 2 (spouse, step-parent to infant) are now siblings! Along with that, the parents of sim 1 are now also showing as the parents of sim 2 in CAS, the family tree, and in the info panel.
What happens when the bug occurs? Bio. mom of the infant became siblings with her spouse (step-parent to infant) after the new spouse adopted the infant. The new spouse also automatically became the daughter of the bio. mom's parents. They are all in the same household. I cannot adjust the relationships in CAS, even with fulleditmode.

What do you expect to see? I would like to see the adopt function work correctly and have it so spouses don't become siblings after adopting their spouse's child, and so the family tree / relationships are the way they're supposed to be post-adoption.
Have you installed any customization with the game, e.g. Custom Content or Mods? Yes, but I have never had this issue prior to the new infant update.
Did this issue appear after a specific patch or change you made to your system? Yes - the latest infant update.
Please describe the patch or change you made. The adopt infant interaction that came with the latest update.

    I've also had this issue - with the Robles family. There has been no cloning whatsoever, and I only noticed it right as Doli died and it said that Jay's *sister* had died! I was so bummed that it was basically too late to even fix the issue. So no, it's nothing to do with cloning - at least, that's not the core issue...I've never once cloned any sim, lol! Unfortunately, I don't know if this happened right after CAS editing, or if it was while I was rotationally playing with their adult children. Given the other comments, my best guess is that it was after going into CAS (I almost always have fulleditmode on, as well).
