Did you try moving the cake to a different surface ?

- Yes, nothing was working. But I think I did find the issue. I went back to a previous save before the party. This time I baked a brand new cake during the party. When the adult helped the toddler blow out the candles, it worked correctly on the first try.
The issue seems to be the cake. I had my sim bake cakes when she first achieved the aspiration reward where food doesn't spoil. We put birthday candles on top so no one would eat them. They've been in the fridge for some time. For some reason this seems to have caused the action not to work with one of those birthday cakes.
I will come back if I can confirm this with the next toddler birthday party.
