There seems to be more than one issue reported in this bug report. The bug report template is designed to house information on a single specific issue/bug and it becomes counter productive/difficult to identify the cause, track the individual issues and advise on each one when they are meshed in together with one and other.

Could you pop open a bug report for each separate issue please? I'll lock this here in the meantime.
