Do you have a large number of sims in that save? If so the localthumbcache.package file gets too big and need to be rebuilt. Only solution is to cut down on the number of sims.

The save on the picture was from a brand-new save. I don't think this is the same issue as the localthumbcache file regenerating due to file size since when that happens, it would regenerate ALL thumbnails including Household thumbnails in the Manage World screen, mood thumbnails, outfit thumbnails, etc.
The issue I described is exclusive to the Relationship Panel. The localthumbcache file is also not deleted. The game just refuses to use the Relationship Panel thumbnails that already exist from a previous session. It's worth noting that this issue only started happening after the Life and Death patch which introduced the unique thumbnail for the Macabre trait on the Relationship panel.
@Kimisadechino How big is your file ? Did you refresh the whole folder or just the saves folder ? If you haven't deleted the file, it might be good to do that.
I did a folder refresh and a game repair. The save on the screenshot is new and is only 8 MB.
