I've had this happen very infrequently with both my own sims and at least once with a service NPC (maid). Unfortunately, I use mods and there's no way to predict when it's going happen, so I can't share a save. But, the last time was just a day or two ago: the maid suddenly left the house and walked a couple of blocks away to... go fishing? Never had a maid do that before. When it happens with my own sims, the sim suddenly leaves the house to go all the way across the neighbourhood to speak to a rando or "See what's happening." (Spoiler alert: There's nothing happening!) The only way to stop the sim from wandering off is to reset them.
