Sudden sim death

What expansions, game packs, and stuff packs do you have installed? Get famous, for rent, discover university, pets, seasons, cottage living, island living, snowy escape, spa day, desert lux
Steps: How can we find the bug ourselves? Just fix this buggy game
What happens when the bug occurs? Sim just disappears, no warning, only find out he died through the moodlets of the remaining sims of the household. Grim reaper doesn’t appear either. Can’t find thumbstone or urn anywhere in house or household inventory
What do you expect to see? Warning sign and music, grim reaper appears, sim dies, then tumbstone or urn is left behind
Have you installed any customization with the game, e.g. Custom Content or Mods? On console - never used.

My sim was talking with his wife. He just disappeared from screen and household. No warning or music played. Grim reaper never appeared. I only found out he died through the moodlets of my remaining household. I can’t find the thumb stone or urn anywhere on the lot or inventory. I have no CC or mods as I play on console
