Sudden sim death

Product: The Sims 4
Platform:PC
Which language are you playing the game in? English
How often does the bug occur? Rarely (0% - 9%)
What is your current game version number? 1.72.28.1030
What expansions, game packs, and stuff packs do you have installed? All packs made to date
Steps: How can we find the bug ourselves? I honestly don't know
What happens when the bug occurs? My sim just suddenly died. No warning, no reaper, just disappeared and became an urn and the sim husband is sad.
What do you expect to see? My sim mom was painting on the digital paint pad, she reset then disappeared and became an urn. My sim shouldn't die from painting on the digital pad.
Have you installed any customization with the game, e.g. Custom Content or Mods? Never used.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure

So this is super weird. Sim mom and dad, dad is doing yoga in the yard, sim child sleeping after mom read her a story. Sim mom goes to paint on the digital pad, she resets ( T position) then becomes an urn. No reaper appeared (they all have death flowers at "perfect" to avoid death). Sim dad didn't stop doing yoga but now has the Widowed and Sad emotions. I don't even know where to begin to try to figure out what happened.

    Hi! There is not a burnout death indeed.

    She came from work (I had her work hard) with no extreme emotions, i hadn't even queued up any interactions yet. As soon as she stepped into the house she was just gone. I was even reporting this as 'my sim disappeared' because there was no animation, no grim reaper, and no apparent reason for her death.

    I also did not receive a notification about their death, i got a burnout notification and she vanished from the UI (the burnout is probably real tho)

    My sims keep dying and I get no notification or grim reaper, they just suddenly disappear from my household and an urn is there. I have not downloaded any new CC, mods, or DLC science or directly before this problem started about a week ago.

    Yeah, it's the strangest thing my sims don't even react to the deaths either. Thank you anyway

    @crinrict This happened to me twice. Once my sim was using a telescope and got the meteor warning. They disappeared once they were under cover. The second time it happened my sim was hysterical and they died and disappeared when I clicked on the mirror to calm down. Grim didn’t show up either time. I have no mods and my game is up to date
