YA sims spontaneously turn into urn with no grim reaper or death scene

Product: The Sims 4 Platform:PC Which language are you playing the game in? English How often does the bug occur? Occasionally (10% - 49%) What is your current game version number? 1.95.207.1030 ...

This also just happened to me, my YA sims went to the toilet while I was focused on another sim. The only notification of the death was the sad moodlet on the other sim. Urn was in the toilet. Not sure how I would even go about recreating the situation.

      @pollylaua Did you check if any setting on MCCC could be responsible for it ? If you have a backup, I would load that. It's really impossible to tell what was the cause and if it was mod related unless you recreate it on a modless game.

      I know OP said they haven't used mods but they haven't come back either and for the it was an NPC so yours might be different.
