Go with the first one, unless, say, a character is going off in a very long story. And even then, perhaps try having other characters make comments in between, or do actions not involving dialogue.

I'm reading the PDF version on my laptop. When reading the first part, which talks about the biology and psychological response of trauma, I was thinking about the Racket-Rotters (and Annette and her crowd, for sure).
