I like the idea of adjusting masculine/feminine body language and mannerisms. There are some Sim ideas I've had but never properly been able to realise in the game such as having a man who has more effete gestures or an out and out tomboy...so when they flirt or freshen up in the mirror they default to their gender. On a related note, not every item of CAS clothing was useable by either gender either and then with the masculine/feminine frame option for the opposite gender even more clothing became unusable. By my reckoning I must have created over 100 different Sims and I did try to create a cross-dressing one but flirting whilst in drag by doing the muscle flex didn't look good. Oh the comedy value was there but that's not what I was going for.
As for more mental health options, I'm not sure. This is a family game and depicting more realistic mental health options can lead to some very dark places for gamers who may suffer from them. Yes, there's the depiction argument but not everything has to be depicted in a game that has to appeal to a very broad audience.
