For me personally, there are four areas in this game that I feel are either not nearly as well developed as they should be or missing altogether. In rough order from highest to lowest priority they would be:
World Size - This has been my chief complaint from day one. The worlds in the game are way too small- to the point that calling them 'worlds' is somewhat of a misnomer. They're a significant step back not only from 3 but arguably even from 2, and what we're left with barely gives us enough to do and explore in an in-game week let alone a sim lifetime. And of course, seeing as there is no open world (which is something that would also be nice, but is ultimately not a hill I feel strongly enough about to die on) the world sizes are particularly frustrating because, if anything, that should allow for larger overall worlds, not smaller.
(Lack of) Story Progression - It can even be the 'lite' version of what was in 3; there just needs to be something that prevents npc sims from being completely static. At the very least all sims in a world should have the ability to independently make relationships, have children and seek/change jobs and homes.
(Lack of) Adjustable Lifespans - Some of us like to play the long game- well, I can at least say that I do. As someone who tends to play the Sims in a roleplaying fashion centered around a single individual at a time rather than as a simulator for a whole multi-generational household or neighborhood, I really wish 4 had 3's adjustable lifespans- or, at the very least, some much longer pre-set options than those currently available.
(Non)Active Careers - The active careers that have been added in some of the expansions have been great but now that same active gameplay element needs to be added to the base careers as well. Make it optional if need be for the player's who might find that type of gameplay monotonous, but at least make it available for those of us who actually like to see and experience everything their sim is doing. Modders have done some great work but this should be a vanilla feature plain and simple.
Those are the major items I would consider to still be 'missing' in the sense that they are things I would expect at the most base level of gameplay. Certainly there are plenty of other things I would like to see added but they are more along the lines of things I would have always expected to be more expansion material. Seasons/weather and school related gameplay features are the two I'm most eager to see in that category. And obviously pet content - albeit something I don't personally make much use of - such be near the top of the list as well just due to the prominence such animals companions have in the overall human experience.
I also agree with a number of previous posters that more careers and hobbies are needed down the line; I would add additional skills as well. I'd also like to see major expansions of the trait and emotion systems (which I actually consider to be some of the best parts of the game but nevertheless ones that still need further fleshing out). Having a 'Fear' emotion, for example, is a necessity and should arguably be a fifth point on my list above.
