I don't know if it has been done before in a previous game but I would love to see a complete overhaul with hospitals, illness and general healthcare and wellbeing. I know that Get To Work added in becoming ill and a doctor career but I feel it could be taken so much further!
For example, it would be good to see both hospitals and smaller doctor's offices. As a child sim, maybe you would have to go down to the local doctor for a checkup or something. Then maybe your sims could get sent to the hospital and treated.
I feel like this adds so much potential for more interesting gameplay. As a realistic player, this is something I would like to see as either a game pack or an expansion pack.
