I stack debug items now (so maybe this post is no longer reflecting current behavior?) – though it works with varied results, depending on the item. I don't use Mods, though I do use the MOO cheat to do it. However, I'm avoiding this in my current builds since there appears to be a bug now where doing this currently may cause an issue:
