@Aceeee82 is this happening to your Sims or is it happening to Townies? I can think of a few reasons.
Because with Townies they all have wierd random outfits when they are generated in my game, there's a bug report here:
So I delete some of the new ones that are generated anyway (because they cause lagging on console) but with the Townies I want to keep I go into CAS and edit all their fashion choices so they look presentable.
If it's a Sim you crested that you are currently playing and if you didn't create that outfit for them then that shouldn't be happening. For your Sims I'd go through their fashion choices set for them and check that outfit isn't there in CAS.
It can happen with a Sim downloaded from the Gallery though if it hasn't been spotted. I don't download Sims now but I did try it once years ago and he kept changing into underwear to go outside, I realised he had 5 outfits per category and I'd missed the one that was just boxers!
@simsplayer818 It's an original sim that was born in-game not downloaded from gallery and it's a sim I play.
I created his outfits he has no outfits with hats.
He just spontaneously changes to the same outfit with hat and headphones.
This happens to two of my sims on different households. so strange...I'd hate to also solve this one by myself as I have zero ideas. helpppp!
@Aceeee82that definitely shouldn't be happening. Are you playing with mods/cc? If so then the advice is always to test in a vanilla save.
Edit: these suggestions are highly doubtful since you dislike this outfit so much but just to check it's not a trend you've created with a Style Influencer Sim, an outfit worn by a celebrity (Get Famous) Sim you've played with that outfit who has a fame perk that makes Sim copy their outfit or an outfit you've created on Trendi (HSY) for sale.
@simsplayer818 It cannot be none of the above.
The problem is not the outfit, its the hat&headphones. It's a CAS Item. Whenever the sim spontaneously changes, he stayes with the same outfit he has on (Could be everyday, formal, party, hot or cold. Any outfit except sleep) and add the hat with headphones.
Those are different outfits but they belong to him and not a trend or copying others.
- @Aceeee82 I didn't think so but just covering every base I could think of. I know the hat you mean, I use it with Athletic outfits on my celebrity Sims. My Sims often randomly change into an outfit created as a trend by one of my Style Influencers and I wondered if that was a possibility.
What about mods/CC in your game? Are you currently using those at all? @simsplayer818 I turned them off and deleted localthumbs to see if there's a change but no luck.
@Aceeee82 Try moving your mods folder out to your desktop, sometimes turning them off in your game options doesn't fully remove the effect of the mods. Depending on what mods you use, you should also check your saves folder to see if there are any mod settings files there.

I'm pretty sure its a gameplay related issue and not a mod related bug (That's why I posted here and not on the bug forum).
Something those sims did/have. I tried checking all their traits and fashion likes (they have none) with no luck.
We need to get creative. I solved a problem before by checking traits its always useful.
I've only been playing for several months so I'm not familiar with all game features and quirks...
Any help would be very much appreciated.
@Aceeee82 Do you use MCCC? If so, what happens when you use MCCC to change their outfit (the option is on the sim, I believe under MC Dresser, though I'm not currently in my game right now).
