@davethevampire Hi! I experienced the same issue recently. Are you using the Wonderful Whims mod (or it's counterpart; we're not allowed to discuss the specifics of that one)? If so, just click on the sim, click the personalization menu, click the clothing option, and then click Change Sim Underwear. This will take you to CAS and allow you to choose the appropriate underwear for your sim.
