Oh we do have that expansion I thought the alien one was one by itself I didn't realize it was part of the Get To Work expansion. Our characters aren't aliens but it does make me wonder if it's something related like maybe a mod did something that may have overwritten our every day outfit section with something in the alien section that for some reason imported accessories from our career section lol I don't know, it's so strange that CAS can't even fix it.
That picture is like what we see on our Sims now for every one of the every day outfits except for we have a bracelet and necklace. Sometimes we can get a 2nd bracelet, hat, or glasses too on outfit slot 2, 3, 4 or 5, but never any pants, undies or shirt.
I've uploaded the video to Youtube to show this bug in action. I censored the bottom part though there's nothing shown anyway. Still, in case someone has never seen a barbie doll with no pants and gets the shocked reaction like Sims do, I don't want anyone to die of embarrassment! The outfits all look correct in CAS and you can see in the video they look fine in the outfit chooser until you're wearing an every day outfit then that outfit changes to the messed up one even in the outfit chooser. It still shows correctly in CAS though. For a while changing to the everyday outfit would mess up the entire outfit list showing all of the every day outfits messed up. Now it displays them fine in the chooser except the one you're wearing but still has each outfit messed up once you wear it. You can see the other outfits in the other categories work fine. It's just the every day outfits. This was with no CC and no mods.

Maybe this video can help figure out what caused it and what to do about it. I wish there was some way to edit the save file, I'm still looking into that too. Maybe it could be fixed that way. Or at least just reset all outfits back to random default ones clearing out anything leftover. Not sure that can be done though.
I just want to say sometimes can a mod (normally only script-mods) backed in your save game + sim. Is especially rare and think this modders earn a reward if they achieve this. If this is so, your only option is to delete the sim/savegame and make a new. Have you tried to make a twin from you bugged sim?
Ah... another question: Your sim had only 4 create outfits in his everyday or? I encounter that if you have 5 outfit, and this sim joins a club with a club-outfit that your outfits ... can break (this occur too in a vanilla game) Since then, I give my sims only 4 outfits for each section if they join clubs with club-outfits.

In the video I had 4 outfits set up because I had been deleting and adding outfits testing stuff. I think at the time I first noticed the bug I only had a couple set up in the every day section. One of the sims in our household was brand new to the household too so she only had 1 or 2 set up as well. I'm fairly certain our other sims didn't have 5 set up when the bug happened but I'm not 100% sure about that. Not sure if we set up a club outfit. I don't remember doing that but she may have so I'll check in game.
I did try a few more things last night with the sims. I noticed when using some tray file utility that lets you track down cc used on your sim (we had none listed) that in the long list of items that I'm assuming were all outfits use by our sims the yfBottom_Nude thing was there even though none of our sims are actually set up in CAS to use it other than the bugged outfits. I didn't see other body parts listed as outfits in there but could have just missed them and this could mean nothing.
Another thing I noticed in testing last night is like what you suggested, making a twin. Several days ago when I imported a sim from the gallery someone had made I saw it wasn't affected, so it's specific to our current sims. Last night I separated an affected one one from the household, saved it to the gallery, then imported her back and the newly imported one while she lost all of her relationship status was not bugged. She did keep her skills and all of that though. I tried that with the entire household I had to delete nearly all of them first though since we had 6 so importing all 6 would be more than the 8 allowed. It did seem to work though! They lost all of their relationships though with all the other NPC sims and each other.
One question I have about doing that is are there any negative consequences other than losing the relationships that I can fix pretty easy? I saw their skills, fame, and traits and all of that were fine but we're pretty new to the sims 4 so I may be unaware of some other things that may need to be fixed. Before doing that and committing it to a save I want to make sure whatever I may need to adjust is something that can be adjusted.. If it's just a few minor things that I can fix in the cheat console or with MCCC then we'll definitely do that so we can get back to playing again.

So, I've imported Sims from one save to another.
Basically: They keep their skills and any aspiration reward traits, and relationships with anyone in their household. They lose their household and personal inventories, and relationships with anyone outside their household. (Yeah, we lost our death flowers and angelfish, but their save was totally bugged from a bluescreen while saving.)
You can save multiple households to your tray, and multiple houses, and then import them and cheat their relationships back. Some people create a room in the house to put all the inventory stuff.

Thank you for that link and reminding me about the inventory! I would have definitely forgotten about that. That site seems to have a lot of other good information on it too, going to definitely look around on there a bit for more tips.
Also, thank you both @xochiquetzl_xkvn and @daikoyu for being so helpful! It's fixed now and I am 100% sure without you two helping us with things to try and ideas of what might have caused it we'd have given up by now and probably reverted to an old old save AND probably had the same problem happen again!
While I was importing the household back into that world I left the club leader as the only one left that was bugged so I didn't lose our club and all the club traits and stuff like that. As soon as I added the twin to the club it bugged her. Then I remembered what was mentioned about club outfits. I knew we hadn't been really using those but it made me wonder if we had ever set one and if we had what ours was set to. Wow it jogged my memory big time going into there. The same exact bracelet and necklace were the only 2 items in our club outfit still. It hadn't been causing a problem at all until a few days ago. We had been wanting to see if it would just add that to the outfit we had on at the time and I guess when it didn't we just always used the every day outfits or other outfits anyway. Maybe when we were copying outfits around we hit that 5 outfit limit on the every day and it bugged that category forcing the club outfit there. Even tho we never kept 5 just having 5 briefly when getting some new CC To try out and copying stuff around may have bugged it. I'm not sure.
As soon as I removed the club outfit and set it to None in the club settings all of our sims were instantly fixed. I feel like an idiot for not checking that earlier but we hadn't even used the club outfit and didn't even finish it so we had totally forgotten it even existed.
So thank you both so much, it's nice to be able to play again and know what caused it.
