Oh we do have that expansion I thought the alien one was one by itself I didn't realize it was part of the Get To Work expansion. Our characters aren't aliens but it does make me wonder if it's something related like maybe a mod did something that may have overwritten our every day outfit section with something in the alien section that for some reason imported accessories from our career section lol I don't know, it's so strange that CAS can't even fix it.
That picture is like what we see on our Sims now for every one of the every day outfits except for we have a bracelet and necklace. Sometimes we can get a 2nd bracelet, hat, or glasses too on outfit slot 2, 3, 4 or 5, but never any pants, undies or shirt.
I've uploaded the video to Youtube to show this bug in action. I censored the bottom part though there's nothing shown anyway. Still, in case someone has never seen a barbie doll with no pants and gets the shocked reaction like Sims do, I don't want anyone to die of embarrassment! The outfits all look correct in CAS and you can see in the video they look fine in the outfit chooser until you're wearing an every day outfit then that outfit changes to the messed up one even in the outfit chooser. It still shows correctly in CAS though. For a while changing to the everyday outfit would mess up the entire outfit list showing all of the every day outfits messed up. Now it displays them fine in the chooser except the one you're wearing but still has each outfit messed up once you wear it. You can see the other outfits in the other categories work fine. It's just the every day outfits. This was with no CC and no mods.

Maybe this video can help figure out what caused it and what to do about it. I wish there was some way to edit the save file, I'm still looking into that too. Maybe it could be fixed that way. Or at least just reset all outfits back to random default ones clearing out anything leftover. Not sure that can be done though.
OK, yeah, so... have you tried deleting your localthumbnailcache.package file out of your The Sims 4 folder? (Documents -> Electronic Arts -> The Sims 4)
I would suggest deleting that file and seeing if it helps. If not, maybe delete the file again and try with no mods or CC. If that fixes it, you can re-add your mods in small batches so you can figure out if one of them is causing the issue. (I've also heard of people putting in half their mods at a time, which is probably faster.)
I recently had an issue where (LOL) all woohoo resulted in babies, even with elders, despite not having mods set to do that, and deleting the localthumbnailcache.package seems to have helped. (fingers crossed!)

Ty both of you for the suggestions and help btw! Having a couple others see the bug and give ideas of things to try is very helpful in tracking down something like this. Being pretty new to the sims it was feeling a little overwhelming without us knowing whether or not anyone else had experienced stuff like this recently or having much to go on when trying to track it down. Seeing the alien sim have that happen an the suggestions were very very helpful!
I did delete that localthumbnailcache.package before a few times and sadly it didn't fix it either. I also removed the entire The Sims folder in the Documents section to reset the game entirely, then moved the 1 save back and it was still affected. It seems its something stuck in our save files attached to our sims.. Since it looks like it's just our household sims in those save files and not new ones imported I think the save is sort of fixable doing what was tried last night with saving our sims to the gallery, deleting them in game and importing them from the gallery one by one. That seemed to fix their outfits when I tested that last night and kept their skills and other traits and stats. It did mess up all of their relationships though. I'm not sure what else it would mess up so I haven't done that then saved & started playing that save yet. So we may have something that will work. As long as whatever it messes up importing them again is fixable by MCCC or in the console we'll probably do that today so we can start playing again.
Oh wow, all woohoo resulting in babies would be a funny bug to have but I can see that getting out of hand quickly lol We've found some funny bugs like that, and a really funny one on XBox. The xbox one I guess wasn't a bug more than it was sloppy typing when using the outfits.copy_outfit command that caused her sim to go to work wearing absolutely nothing at all. It was an embarrassing day and her sim was not happy when returning from work. It was easily fixed in CAS but once we figured out what she had typed we had to play with that a bit more in other outfit slots and had some hilarious interactions with NPCs and the things they would do over the course of a night when the other person was sleeping without pjs. The house was cleaned, several pots of coffee were brewed and the computer was broken all in one night by her soulmate Liberty who just couldn't handle being in bed for more than a minute or two without being shocked and getting up to react lol After having many laughs trying that out she had to fix the outfits in CAS or it would've been a hard to have a normal game again. That or we would've both died of laughter IRL watching the soulmate NPCs run around the house like crazy exercising, cleaning and brewing coffee every night.
