Children keep dying, then neglected but not in manage households

This just happened in my save without mods. Said she was being whisked away due to neglect than the pop up for death came up. She's not in manage households now. Pretty sure she was only marked as neglected because she had an F because I had to age her back down to a child after she aged up twice (according to searching other bug forums). I have every pack except the two newest creator kits, if that helps any.
