I think I found the solution for your problem. There's a way to adjust the User Interface size for the Sims 4.
How is explained in this thread on MTS.

Hello It is very kind and thoughtful of you to post a link to that thread :)
There has been simmers though that have found that by using the method in that link, it has caused issues with photos that sims have taken in-game using the camera. If you do use that method, it would be best to test to see if the photos that your sims take have issues after doing this.
  • hi ty for replying i realise that when you have a famiy of 8 the family heads do go smaller but mine go so tiny its hard to click on the right sim to see what they want ive watched youtubers play with a family of 8 and there heads dont go as tiny as mine unfortunaly i cant take a pic and show you all the problem im having =]

What resolution are you playing the game at? The portraits will be smaller on higher resolutions.
  • Try 1920 X 1080 and see if that helps. You should still be able to play full screen.

This would make it worse. He needs to try reducing his resolution, not increasing it. Reducing it will make the portraits larger.
