I have Neighborhood Stories on for unplayed sims and off for played sims. The only time I've gotten babies in played households that are under the My Households tab of Manage Households is when I have either said yes to one of the "Should I have a baby" phone calls or when I used the "Discuss Expanding the Family" social when talking to a sim not in my active household. So if you did either of those things, that could be the cause.
Or if you play with mods and one or more of them have options that affect pregnancy, that could also be the cause. If you do, then disable any features that affect pregnancy or pull those mods and play for a while to see if anything changes.

Thanks, but I’ve specifically never used those interactions (I even had one family phone about advice for trying for a baby and I made sure to click don’t!) and I don’t have any mods. I think I must have a glitch.

Just making sure. Under which tab in Manage Households are the ones that have been having the babies? My Households or Other Households? Sims that you have played but are now under the "Other Households" tab will be subject to the Neighborhood stories actions. If you've moved a household to Other, they will be seen by the game as unplayed. You can tell by the color of the plumbbob icon.

This is possibly a dumb question, but... are you using any mods with Risky Woohoo? I did get a surprise baby in one of my played households after rotating away, but I also have risky woohoo on, so I blamed risky woohoo and assumed I just forgot to check and see if she was pregnant before leaving.
