I had the same bug. I used Sims Tray Importer. Basically I created a totally naked (no clothes, no hair, no accessories, no body marks etc) male Sim, who of course was wearing a bra and panties, and saved it in my gallery. Then, I opened sims tray importer and clicked on said Sim. Then, I clicked on the custom content section and it showed me two pieces of CC. I deleted them, went back to my game and everything was solved!
Brilliant!! This has been cursing me for MONTHS and I had resigned myself to all male sims wearing tank tops in the pools lol. Thank you!
That didn't work for me. I removed EVERYTHING and deleted the mods that it claimed were the problem and redownloaded updated versions of the mod. The game will force me to do this. I think its a corrupted save file.
Are there any other options or ideas? I think I might have to completely purge my saves and restart every last one of my Sims to remove this bug.

I believe it was a corrupted save. I had backed up my files and now the issue is gone after I loaded them. I ran the mod you suggested and then loaded an older save when that didn't work and deleted the corrupted save. It appears to now be gone when I try to look it up in my library and gallery.
And after I posted this it is back. Something has corrupted my files again and I'm not sure what it was. I tried in a game that I haven't used in over a year for build lots only, and the males all have bras instead of bare chests. Something messed up all my saves so I have to completely redo ALL of my Sims on a fresh game now...
@Crdroxxpl The problem is caused initially by broken CC, but you'll also need the "fix" mod to remove that bra. Instructions for the 50-50 test method to find the problem file are linked there.
