Sim died but suddenly vanished with no urn or from reaper

Hello,

I had just gotten my sim couple married. Came home to have the wife sim build up her followers for her career. I accidentally made her hysterical and she was dying. But she just vanished before she even collapsed. There is no urn, there was no from reaper, and suddenly she's not in the household.

I have no urn to try and do anything about her spirit. I have no from reaper to plead to.

What is going on? What can I do?

When there are no one else in the house there is no reason for the reaper to come as no one can plead for her life.
Look in Household inventory for her urn and what else she had in inventory.
