sim holding head with blue smoke and strange noise

Hey guys, I need help with something. Can anyone tell me what would cause a sim to hold their head, with blue smoke around it and a strange/eerie noise playing for a second? I'll add a picture to sho...

Is it because she an alien and mortified? I've never seen it happen to any of my sims before and plenty of them have been mortified teenagers.

No it happens with all mortified sims.
