I was having the same issue in my game and thought it was a bug, but after doing some mod testing, turns out, like someone else said it was the mod, No Less/CL/IL/SE names by simmytime that was causing the issues with the last names. It has since been updated for the Cottage Living patch and my sims have their last names back.
I still have this error after updating the Less CL Names mod.
Finally found out the issue. The More Randomised Townie Names mod also causes this problem.
I've updated both mods and have no issue with this anymore.I also have this bug ever since Cottage Living came out but I have none of the mods mentioned. I removed and updated my mods but my townies are still born and generated without a last name. Even when my active household sim gives birth, the baby doesn't automatically have a last name, but I have to type it in.

I have this exact same issue. None if the mods above or other name mods, was told to set mccc back to default and did so, 2 new random townies without last names in a fresh test save file :/ I'm waiting for townies to have kids now via NS and see if they get surnames but I doubt it
