That's a very common issue and not necessarily related to Catalina. Move your Sims 4 folder to the desktop and try to launch a clean game.
Thank you for the suggestion. I tried and it had no effect beside the fact that now the window is black.
Maybe I wasn't clear enough on my original.post but I can't even get the menu to load so I can't start a new game. I click on the play button on origin and the usual game window opens, with the difference that it's all white -or now black- and doesn't load the start menu, nothing at all. (I'll edit my original post with this info to avoid confusion)

Please run the Origin reset tool then try again, not the diagnostic tool that I think you were referring to in your original post?
The point of moving your Sims 4 folder to the desktop is to rule out any issues with your user data such as saves/mods etc.

oh my! yeah I didn't expand and didn't see the reset tool but downloaded the diagnostic one instead 😞
I'll run that and try some of the methods suggested on the linked page.

So I've downloaded the origin reset tool, but before running that I've decided to try and repair the game first and it seems like it worked.
