Hello! I have FINALLY (at least seemingly so) fixed this issue, I'm not sure why no one from EA or The Sims team haven't posted about this as it was a fairly easy solution. I have only tested this on the new EA App for Mac, not in Origin.
My problem was slightly different than OP's, My refresh rate would drop to the lowest value but also the resolution would switch to 720P and the screen would subtly flicker when moving the mouse in full screen. This made the game unplayable in full screen.
RESOLUTION:
You need to go into the game properties by hitting Manage>View Properties
Once there simply enter the following command into the advanced launch options: -fps_max_144
TIP: You can change the number at the end of this command to represent the refresh rate of your monitor for me its -fps_max_144, for OP it would be -fps_max_60
This resolved the refresh rate and flickering for me entirely! it also prevented my resolution switching to 720P automatically. I hope this information will help someone out! ❤️
