I don't play Sims Medieval but I do know that EA used Cider to make it Mac compatible. Cider was a tool used to 'wrap' the Windows version of the Sims 3 so it would run in macOS, it was never natively written for Macs. Unfortunately this created a lot of the gameplay issues, one of which is the error message you are describing. For Sims 3 I would suggest completely uninstalling and reinstalling again. Whilst I haven't written instructions on how to do this thoroughly for Medieval, the locations will be the same for it I should imagine. Read through this guide and look in the same locations for Medieval files.
The other issue you will have is that your Mac is much newer than the game and as a result is running an unsupported version of macOS and has an unsupported graphics chip - Intel Iris 550. You can manually add the Intel Iris to the game files to see if that helps. Again, I have no guide for Medieval but the Sims 3 equivalent will point you in the right direction 🙂
