@jsophiak I've split your post into its own thread since the other thread is a few years old already.
The second copy of the files is likely from OneDrive: it's uploading your data to cloud storage and then redownloading it. The DESKTOP-some characters designation is added to tell you which computer the files came from; if you check your system info in Windows settings, you'll notice that the names match. But with the extra characters in the names, Sims 4 won't read the files at all. And since these are older copies of the files, you can safely delete them.
If you'd like to stop OneDrive from interfering in this way, you can choose not to automatically sync the contents of Documents:
Before doing this, make a backup copy of the Sims 4 folder in Documents > Electronic Arts, just in case. You can stash it in Downloads, which OneDrive doesn't sync, and restore it afterwards if necessary. This process is normally safe, but every once in a while, OneDrive eats the folder or some files inside, so it's better to make a backup.
