@Storxm0 Hi, I'm a player like you, as are almost all the responders here. You might see the occasional EA community manager pop in, but mostly this forum is players helping fellow players. Maybe part of the reason that you haven't received a reply is that you've already tried all the suggestions that people could think to offer. (I'm guessing you've confirmed that this happens in any save, even one with brand new sims in a clean folder.)
Having said that, it did occur to me that you could try experimenting with one fps limiter or another. TS4 doesn't need one, of course, but perhaps its built-in function is struggling or malfunctioning a bit in your case. An outside tool might do a better job of smoothing out your frame rates. To be clear, I'm not saying that it will help, but since everyone seems to be out of ideas, it might be worth a shot. (It's also a possibility that you already have an active fps limiter that's messing with the game, but you didn't mention using one.)
With Nvidia cards, the built-in Control Panel is usually the first choice, but it likely won't work in windowed mode. Nvidia Inspector (free download) would be next up. I have an AMD card, so I use RTSS (another free download), which works great for me, although it does need to be running while you play; MSI Afterburner is also a good option.
