Try scanning every surface with a slot for dishes and manually remove them.
Yes, that's been my experience but we shouldn't have to do that. Neither my sims or butler will do it and when you're in a house of 7, it's just easier and less realistic to just take the dirty dishes to the trash or sink. That shouldn't be the case.

The shelves problem has been patched in June but if you still have stuff on shelves from before that, the issue is not resolved on it's own.
If you're still seeing them put things on shelves, please use the first of the provided links and provide your save.
