As @EA_Barry said, holding ALT & ENTER should work. As for the desktop shortcut, try finding ]thesims2.exe[ or whatever the file is the game runs off on Origin in Windows Explorer, clicking "Create Shortcut" and placing it on the desktop if it doesn't itself.
