When you create a new sim either from a baby or as an adult you need to assign them a bronze orb that you have in your inventory . Then assign them a life wish. Once complete and the sim passes on you will be left with a silver orb. You then assign the silver orp to a new sim to get a gold orb. And so on. You can go here for more information.
