This is odd indeed. Can I ask you to try to perform factory reset on your console http://support.xbox.com/en-IE/xbox-one/console/reset-console-to-factory-defaults and try to get this achievement one more time afterward.

That did unlock the achievement. I'm still not too pleased with this whole situation, as I have had to waste probably 2-3 hours redoing this achievement, and Respawn doesn't even have the courtesy to acknowledge the problem in their own bug report forum. This is incredibly disrespectful of the player's time and effort.
