Select iPhone
Select iTunes Store
Select Connecting to iTunes Store
Answer OS, Version, Country and Describe as Apple ID is disabled
Enter your Apple ID and Password
Provide your contact info and enter the following note:
I can log on to the Apple website to manage my account and all the information
is correct, but I get Your Apple ID has been disabled when I try to get
something from App Store on my iPhone.
