Tactical and Ultimate not working in ranked.

I've had this issue happen twice to me in ranked where midway through the game, I can't use my tactical or ultimate. This has happened to me twice, both on Storm Point while I was playing Fuse. I am not sure what triggers it but when it happens, it makes the sound as if I hit got by Seer's tactical and tried to use them.
