Matchmaking Issues when playing with squad

Experience recurring issues with matchmaking when squading up with friends. Multiple times we will queue up only for one or two of our squad members not make it through to the match. The squad members who do not make it through remain in the lobby section, and are unable to join matches in progress. This happens to squad mates on both PC and Xbox. The squad mates who then make it through have to back out to rejoin the lobby, where we try again. This issue happens probably a third of the time when we play with a squad of 3 or 4 friends. Very frustrating.
