Tombstone Pack Missing

I preordered the Phantom Edition through Walmart and never received the Tombstone Pack preorder content that was advertised. I went through the full trouble shooting guide with both EA and Walmart.  Now both sides are claiming they aren’t the ones who are responsible for the code. Unacceptable. I paid for advertised content and expect to receive it.
