The date and time (MM/DD/YYYY) (HH:MM AM/PM) that you saw the bug? I've been able to avoid it for about a week now

How often does the bug occur? Seems to only be the first match after turning the console on if that match is a squad battle
The error message that you receive: the standard fail message
I've been playing a friendly before a ranked game lately and it seems to stop this crash
