I know there are 2 more threads showing that LvL 40 rewards is shown auto claimed with auto selection of Roberto Firmino. In my case I have not received any of the 3 rewards.
How it happened, I claimed the Feller reward post which the game went on loading loop. After that I restarted FC25 and went to claim Lvl 40 rewards which now appears as claimed.

The level 40 diamond is still with "Grey" color but it says claimed.
I neither have Roberto Firmino, nor I got 90x4 nor I got the pack. Just nothing, it appears a faulty claim message.
