Approximately one match a game week, only in squad battles and only on non-repeatable matches. This has been occurring every game week since the second week of January. Frequency has increased since the start of February

Same thing here - usually 3-4 times a week. Already twice today, I've reported 100+ times and EA doesn't care. IF they reply, all I get it is "check out the forums". It's a pathetic response from customers asking for help and they get away with it. I hope an EA employee reads this and is embarrassed. I'd be embarrassed working for that company.
