These are just ones I randomly caught, the above situations happen 10-15 times a game it seems. That might be on the high end (7 min periods), but something is way off with the D-Men Logic constantly wanting to leave the zone and lead rushes.
Also, team default strategies of Sullivan from November 20th Roster Update, no changes to them at all.

Is your D strategy set to Tight Point? Letang looks to be in that position. Then, when the carrier skates behind them, they see the open space and start to break out. He could have waited and stayed back, but that would sacrifice the change to get the rush going.
Wither Peterson, the center drops back in front of the net. Peterson steps up to take their position. Nieto gains possession and Peterson recognizes he's the best opportunity to breakout.
