Broken nametags & character models in Party

Often within a party with friends, either nametags break in such a way it will float in a location the person has been, but will not follow the person around as it should. I am unsure of what causes this.

Also, I've noticed after teleporting to a party member, one or all members are invisible to eachother. I've only noticed this after teleporting directly to a party member.
