Just took my R10 Satele against an R10 LV. Have all three omicrons active. Revan got killed by LV, and I tried multiple times to revive him and he never came back. Others got killed, even during his Ult, and i was able to bring them back.

Looks like Satele is just looking for revive_immunity, but Order 66 uses order_66_revive_immunity and not the generic tag.

I can confirm that. In my battle in GAC Bastila Shan was killed by LV AOE right after his ult, I wasnt able to revive her. Couple of moves later JKR died during LVs Ult and got revived. Bastila stayed dead though and could not be revived.
This just happened to me as well, Jolee could bring back part of the team but not all. I have all 3 omicrons on Satele.
