Illum Force Echo 5 Locked!

Summarize your bug Planet = Illum Issue = Databank Entry 5 Not Unlocked?! During the story, Cal has to go to Illum to get a new Kyber crystal; along the way you can find Force Echoes of previous Padawan's experiences. but still; I'd love to see what lightsaber hilt ideas you can come up with this when I first entered the room: but for some reason it hasn't shown up in my databank; which mean I'm now stuck at 96% Illum Completion; and therefore cant 100% the game!
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? Inside the workbench room where Cal finds his new kyber crystal and repairs his lightsaber; there is a force echo along the back wall that should unlock databank entry 5 "the gathering".
What happens when the bug occurs? Databank Entry 5 - "The Gathering" isn't visible in the menu and my Illum completion is stick at 96%
What do you expect to see? When you collect a force echo, there's a small particle/glow effect left behind and the associated entry is unlocked in your databank. When I collected this echo: it gave the voice lines associated with a Padawan finding a crystal; and the menu prompt to read the entry popped up; but when I clicked the 'options button' it wasn't there: thus sticking me at 96% completion...
