Bad respawn location on Dathomir after death, causes infinite death spiral.

Summarize your bug On Dathomir, made an attempt to grab and cross a pipe, a jump I just missed because of my bad aim. The game respawned me in a freefall to death. It repeated that over and over. There was no way to recover from this spawn. Reloading the game spawned me further back so I was able to proceed.
How often does the bug occur? Rarely (0% - 9%)
Steps: How can we find the bug ourselves? On Dathomir (when I am supposed to be). Have all the powers I am supposed to have. Defeated the big bat. Entering the "camp", pulled down the bridge. Defeated the guys in there, pulled the two bridges and climbed the wall. Took the "stealthier way" in. Crossed the "pipe" under the bridge. Took the path to the right, through the tight passage. Climbed the wall. This is the pipe where it happens. Image is screenshot.
What happens when the bug occurs? My game WAS literally locked at this point. There is nothing I can do. Since it drops you repeatedly, have to quit out and reload. LUCKILY the reload puts you at the death of the big bat, and does not keep you in the death freefall. I tried jumping or grabbing immediately upon spawn in the freefall, nothing worked. Jumping changed where I died, in the void below, but nothing got me to a place where I could do anything but die.
What do you expect to see? I expected to be respawned on a platform next to the pipe so I could reattempt this jump.

I tried to replicate the bug, but I could not. Each time I died from bad jumps or dropping after actually making the jump to the pipe, it spawned me in the place I expected to be spawned. I am unsure the first time through (where it happened) whether I actually grabbed the pipe or not. I think I did then immediately fell from the pipe, but cannot reproduce it.
